Terms and Conditions

JUNIOR JUKERS Warning Waiver & Release Participant’s Name (the “Child”) _________________________ & Participant’s Guardian’s Name (“Guardian”) ___________________ ( collectively the “Participant”) This Waiver and Release from tort and civil liability is made this _______ day of _____________, 20____, between the Participant and Apex Training Academy LLC, d/b/a Junior Jukers, (the “Junior Jukers”). This Waiver and Release shall be considered effective on the date the agreement is signed by the Participant for any and all tort and civil claims arising from and relating to the Participant’s participation in any of the training, activities, or events hosted by Junior Jukers (the “Events”). By entering into this Agreement, the Junior Jukers does not in any way admit that it may be liable in any way. By participating in the Events, the Participant will, among other things, perform physically strenuous sports performance exercises that may cause physical contact or harm. The Part
